Nurse Case Manager (Afternoon Shift)

About the role

Cigna Healthcare is looking for a nurse-led Case Manager based in Europe, working the afternoon shift (Monday to Friday, 12:00 p.m. to 8:00 p.m.). The Case Manager supports members with complex medical, psychosocial or care-coordination needs, guiding them from assessment through follow-up with personalised care plans and coordinated providers and services.

Key responsibilities

• Act as the member's main point of contact and coordinate providers, hospitals, specialists, caregivers and support services
• Assess medical, psychosocial and functional needs and develop, implement, review and adapt personalised care plans
• Monitor progress, support safe transitions of care and work with physicians and clinical experts
• Help members understand their conditions, treatments and self-management strategies
• Deliver specialised clinical programmes such as Case Management, Chronic Condition Management, Lifestyle Coaching and Pre-Assignment Assessments
• Maintain accurate case records and comply with professional standards and regulations
• Promote cost-effective, sustainable care solutions

Required profile

• Bachelor's degree in Nursing
• Minimum 3-5 years of experience in Case Management, Clinical Management or a related healthcare setting, preferably international
• Fluent English; French, Portuguese or German is a strong asset
• Ability to advocate for members while meeting clinical, contractual, legal and organisational requirements
• Ability to work independently in a dynamic, multicultural environment
